Worker’s Registration form

Instructions / SOPs for Social Security Card

The registration form must be duly filled and attested by the employer.
A valid copy of the worker’s CNIC, two (2) recent passport-size photographs of the worker, valid CNIC copies and photographs of dependent parents, spouse, and children.
For children under 18 years of age, NADRA Form-B and photographs must be attached.
Sons over the age of 21 years are not eligible to be listed as dependent. Married Daughter are also not eligible to be listed.
If the wife’s CNIC reflects her father’s name, attested copy of the marriage certificate must be attached.
The Social Security Affidavit Form must be attested by the employer.
Test reports to be provided along with the registration application. (Blood CP, ESR, HEP B & C, Urine RE, Chest X-Ray, Creatinine)
Fee of Rs. 250 and Rs. 150 will be charged for Secure Medical card and Medical Book.
A computerized social security card will be issued within three (3) working days upon successful submission of all required documents.
