Worker’s Card Renewal Form

Instructions / SOPs for Renewal of Social Security Card

The Renewal form must be duly filled and attested by the employer.
A valid copy of the worker’s CNIC, two (2) recent passport-size photographs of the worker, valid CNIC copies and photographs of dependent parents, spouse, and children. (In Case of Addition of Dependent)
For children under 18 years of age, NADRA Form-B and photographs must be attached. (In Case of Addition of Dependent)
Fee of Rs. 250 will be charged for Renewal of Secure Medical card.
A computerized social security card will be issued within three (3) working days upon successful submission of all required documents.
